Done in TR2 - Opera House.
There are 9 windows in the stage to smash. You can either use your body or shoot them. None of them are really that hard to miss as they're all within your route to completing the stage (unless you take a speedrunning shortcut).
3 are in the first area, when you jump down into the building with the glass shards, on the right from where you enter the stage.
6 are in the double story room with all the dressers and hangers. You enter this room just before you deal with the sliding and all the fans/vents with the green dragon in the middle of it.
The achievement unlocks instantly when you shoot down the last window.

Missable if you break too many roof tiles so you can't get back if you're already further in the level and didn't shoot the first three. Then there is no way to get back to get this achievement. Except for loading an earlier save file or restarting the game if you only had one save file.

If you missed those three for whatever reason, you can die and restart the level. No need to start a new game.

Tomb Raider II - Opera House (Level 4)
There are 9 windows total with the Bartoli symbol. The first 3 are down to the right from the very start point of the level. A series of jumps/ledges grabs will get you to them but you'll want to grab the Ornate Key from down below first. 1 window as you enter, the other 2 on the left and right of the glass shards.
The other 6 are after you make a series of standing grab jumps back and forth up a tunnel, fighting 3 rats, a human and a dog along the way. Down the hallway are 3 more windows to smash, opening into a large dressing room. Drop down and the last 3 are directly below where you entered.
