How to unlock the TR2 Nervous Wreck achievement in Tomb Raider I-III Remastered Starring Lara Croft - Definitive Guide
Done in TR2 - Wreck of Maria Doria.
This is somewhat luck based but there are some things you can do to increase your chances of pulling this off.
The very end of Wreck of Maria Doria has a stretch of swimming underwater. If you're not sure where the end of the stage is, just make sure to save if you find yourself in a large room with water below, BEFORE jumping down to the lower floor level.
Before diving into the water, I recommend taking out the underwater scuba divers as best you can from the upper levels so that they don't cause you trouble.
On the swim to the end there are three eels which "reside" in the walls that you need to dodge. Try and stick to the ceiling as much as possible and they shouldn't be able to bite you.
The third eel has a few fishys around it who will bite. I would prioritise sticking to the ceiling and HOPE that the fishys miss you as you swim past them, but this might take a few tries.
The achievement will unlock the moment you see the results screen.

Note: I dived from the upper floor to get the achievement and I *believe* (based on the wording and the result of kaitthegr88's attempts) diving from the lower floor (where the enemies were) won't count. Keep this in mind if you're going for the "All Pickups" achievement as there's a few pickups on the lower floor.

I did it a few times from the lower floor and the achievement didn't pop. Tried it once from the top and it worked

I'm less sure about it counting if you go into the very lowest point. I dived from the upper floor initially to unlock the achievement but recorded this on the lower floor for this guide. If someone could make a save on the upper floor and try doing this from the lower floor and lowest point (where I filmed from) to see if the achievement unlocks for them that would be much appreciated.

A little tricky with the moray eels. To get this achievement, you need to get right to the very end of the wreck of the Maria Doria level, before jumping into the water surrounded by glass.
Once at this point, swan dive directly into the pool, swim towards the barrel on the left (this signifies the way you need to go) and follow the path to the level exit. You need to do this without damage. It’s very luck based, but to avoid moray eel bites you can usually just swim over them to avoid the hitbox. Watch out for the barracudas at the end which strike from the right.
