Done in Tomb Raider 2, between Tibetan Highlands and Ice Palace.
There are 19 icicle sets spread across four levels, though Barkham Monastary has none to worry about. I'll list their locations below together with screenshots.
I recommend getting this achievement at the same time as the "TR2 Eco-Friendly" achievement since one of them is on a route you wouldn't take with the vehicle.
Tibetans Foothills
3x: In the canyon right after smashing the ice wall, you'll enter a cave system. You'll see them at the start of this cave system.
4x: Further in this cave system, above a pool of water.
1x: When using the vehicle for this stage, there's a "rock bridge" that has a big jump in the middle which cannot be reached by Lara's legs. Assuming you're beating this stage without using this vehicle, on the way back from grabbing the key and heading back to the hut, you'll come across a small room just before this bridge, with a lever that opens the door to the lower part of that bridge area. The icicles are just above that lever.
Catacombs of the Talion
2x: At the beginning of the stage. Cannot miss.
2x: After killing the first yeti and a snow leopard, it'll be above the stairs where the snow leapard spawns.
3x: Near the end of the stage you'll reach the upper ice lake area, where the Green secret is located. On the right side of this lake above land, you'll find these icicles.
Ice Palace
3x: After picking up the gong hammer that's under water you'll enter a cave with the 4 yetti running around. The icicles are above the coast. I recommend NOT exiting the lake from the coast in case they accidentally hit you and instead just climb out of the water from the right side, killing the yetti first and then dealing with the icicles carefully.
1: In the same cave but slightly further ahead on the left. You'll need to do a careful jump to reach this.
The achievement will unlock the moment the last icicle falls without hurting you.
