This needs to be done on the fourth level, Opera House.
Keep following the path to continue the level up until you find some wooden panels and a veeeeeery deep fall.
If you step in one wooden panel, it will fall almost immediately, so you need to be fast and precise on your jumps. Save before you try, this is a very tricky achievement.
You must jump on only three wooden panels, here's the path I followed:

Where the other guide is good, I have found a method which some may find easier to achieve this.
Grabbing a tile doesn't cause the wooden panels to fall which we can utilise to our advantage.
I'll try and write down a text summary of the steps needed but my attached video probably shows the process better:

Long Jump Straight Ahead to 1st Wooden Panel
Maintain Speed and Jump at the last second towards the 2nd Wooden Panel and Grab
Pull yourself up and immediately tap forward just enough to take a single step and do a standing jump to the 3rd Wooden Panel
Continue to jump to safety.

Tomb Raider II - Opera House (Level 4)
You will come across the large broken roof with scattered wooden panels as you progress. With the span of wooden panels being 4 tiles, align yourself roughly in the middle of the 2nd column of panels left to right. Perform a running jump, followed by another running jump to grab the edge of the lower left panel of the upside down 't' shaped tiles. Climb up, walk forward one step (just tap Forward without holding), then turn 45 degrees to the right and make a forward jump to the last wooden panel. Immediately jump when you land and you should make it to the lower wooden surface ahead, climb up the grey block to your left for the achievement.
