Leaving the clip up in case someone else has trouble finding the 2nd lever right away.
Description for this one is misleading, the lever on the opposite side is also required so it's 4 levers in total. If you perfectly position yourself one breath is just enough to make it without taking damage, but taking damage doesn't void the cheevo. It's supposed to pop the moment you stick your head out of the water again.

This achievement only unlocked for me when I pulled the other level as well, which opens a gate for some desert eagle ammo.

Good thing I kept level select saves this time. Will do a speedrun and check if it works that way.

Level - Nevada Desert
After passing a dam worker and climbing up you will arrive at this area. Here is a video of all 4 levers you will need to pull in order to unlock this achievement. Make sure you haven’t pulled any before you attempt this, make a save before you start so you can reload and try again if necessary. You will need to pull all 4 of these levers in one breath if you run out of air, watch your health and equip med packs if needed.
If done correctly achievement will pop once you pop your head up out of the water.
