At the end of Level 6 - The Reunion. When you get to the boss fight the camera will show you a large medipack at the other side of the room. Where Sophia Lee the final boss is located. When I picked up that large medipack the achievement popped for me.
I think you only need specific and not all pickups to get this achievement. However i don't know which specific ones that would be. I did pick up almost every item in the game following this video guide:

The list posted by AxeL R GR is pretty close, but I think the harpoons and uzi clips are slightly off. This is what I found - timestamps are relating to the video guide linked above and refer to the exact time when the item is being picked up.
Fling Rockets (7:09) - There is only one rocket to be found in Highland Fling, and it is on the rocks across the water from the dock. You’ll need to jump off the bridge and do a few more carefully angled jumps along the rocks to get to it.
Lair Harpoons (21:38) - These can be found right near the beginning of the 2nd level (Willard’s Lair), after dodging a spiked wall and a boulder. Go through the crawl space behind where the boulder came from to find a pool with 3 bundles of harpoons in it. (I’m not sure if you need to grab all 3 for it to count for the achievement, but you might as well just grab them all while you’re there anyway.)
Cliff Grenades (39:14) - These can be found in the ventilation ducts near the beginning of the Shakespeare Cliff, just after going down a shaft with a couple of drills following you if you’re following Stella’s guide. You’ll need to shoot out a couple of grates to get to them, and they are on the ground behind a large fan. The video shows a different method of getting them, which would be done before going down the drill shaft.
Sleeping Uzi Clips (1:18:01) - These are the very last set of Uzi clips available in the level, and can be found at the far end of the mutant fish tank opened by using the lever in the dolphin tank. They are right at the very end of the level and somewhat difficult to see on the floor of the tank.
Mad Eagle Clips (1:33:32) - These are found in a small room at the top of a ladder behind a tree in the aviary. When you enter the aviary, there are three trees on the right and the ladder is behind the middle tree.
Reunion Large Medipack (1:43:31) - This is the first large medipack available in the level, on the side of the room where the boss starts. You’ll have to jump over to it while under fire and quickly make your way back to the side you started on. (Again, the video guide shows a different method than Stella's guide does.) If you’ve gotten all of the other items, the achievement should pop when you pick up this medipack.

I also got it by luck. I went for all secrets but not all items. Without an achievement progress tracker it's hard to say exactly which items this is about but I'll try to make an educated guess based on the description. Here are the timestamps for the video guide:
level 1 - rocket - 6:52
level 2 - harpoons - 21:16
level 3 - grenades - 39:00
level 4 - Uzi clips - 1:15:50
level 5 - desert eagle clips - 1:33:06
and the last one is the large medipack in level 6 as you already mentioned. Hope this helps.
