Done in TR3 - High Security Compound
This is very similar to the "TR2 The End Doesn't Justify the Means" achievement, where you need to keep all Monks alive since they fight for you.
Only in my opinion this is significantly easier to achieve as there's far less enemies and most of them don't have guns.
I recommend doing this on NG and not NG+, as the enemies will have less health and you can save without limitations. Save regularly and keep an eye on any prison fights that happen - if one dies, just load and try to have the guard fight you instead; you can use medikits, the prisoners cannot.
Of course, it goes without saying that when you get your guns back, don't shoot the prisoner that's in the vicinity. Also try to keep all the enemies that are at the top of that section up there, since there's so many, that prisoner will likely get killed quickly.
The achievement unlocks at the results screen for High Security Compound.
