How to unlock the Lights Out achievement in Tomb Raider - Definitive Guide
I'm sure you could boost this in a Private match, but in a public match I found the best way to get it was:
Select the mode 'Free For All'
Select the Type 'Casual'
When the game starts, just keep looking for opportunities to use the 'Charged Melee' attack, which you instigate by holding Y.
Doing so causes your character to sprint with their melee weapon held high and connecting with an enemy is an instant kill.
Look out for enemies with their back to you and even consider doing charged melee's around corners on the chance that you'll connect with someone coming the other way.
I can't be certain but it also felt like Charged Melee enables you to run longer than the sprint button does!
ADDITIONAL TIPS FROM THE COMMENTS:
RainbowPooper has added a useful tip: "In rescue you can finish downed enemies (if you are the Solarii) with Y when you are close to them. It will count towards melee kills too."
NOBLEPX672 has added that Rescue may be the best mode to try as you actually have to kill the enemy with a melee attack.

I think Rescue is more comfy. Because you actually have to kill the enemy with a melee attack, got it in about my 3rd or 4th match of Rescue. So I would assume if you have problems on FFA (like me) try Rescue :)

Depending on how you fight your enemy he will fall to the ground and this is your best chance to kill him with a melee attack, another way would be to do good damage with your main weapon and then surprise him with several sequences of melee attacks.
To kill someone with with a melee attack, press . You can do this behind them for an instant stealth kill, or swing away at them until they die. If you hold the button, you will do a charged attack.
Online Boostable Achievement
This can be done to the same person if you want, over and over again. You can boost this in a private match very easily.
Press to use your climbing axe and deal a melee blow to your enemies. You can hold the button to execute an automatic attack that will kill anyone in your path or stand behind an enemy for a stealth kill.
An easier way to do this is to play Rescue on Solarii side. Solarii need to kill Survivors with a melee strike to win the game in this mode and each kill will count towards this trophy.
This trophy can be boosted in private matches. Please check Boosting Thread (PS3) or Boosting Thread (PS4) to find partners.
