Torment: Tides of Numenera

When you get to the Lair of the Changing God as part of the "The Cold, Calculating Jack" quest, there will be a set of mirrors in the middle. Interact with them and then choose to interact with the small mirrors. Your next set of choices will include the option to pick the small sphere out of the mirror. Do so and the achievement will unlock.
