This game and this particular achievement have a reputation of crashing. To the best of my knowledge, this has been resolved. Playing on a series S, I had 0 crashes and no issues with the game not starting.
The description is pretty clear that no saving can be done. I would recommend doing this achievement after completing that game at least once. The speed-run record is around 53 minutes and playing safe you should be able to do it in about 2-3 hours. I would suggest further that you attempt this as your 3rd playthrough after completing the NO HEAL run.
I followed the speed-run video below ONLY for path/order. The big difference is along the way you want to grab ALL healing items, ALL ammo/weapons and typically kill all enemies.
-keep yourself healed ALWAYS after taking 1 or 2 normal hits
-attacks from the traps or the floating lady can be more devastating
-IF you hear the floating lady's music exit the room and come back
-explore rooms for health/ammo
-pick up the stun-rod in the area where you repair the guy's missing arm
-save about 7 stun charges for the fight just before the final boss
-save about 20 nails and 6 shell for the final boss
-watch the video for the quick and easy way to kill the final boss

Hello if anyone ends up doing this achievement and you want to do it with other achievements like no heals or under 3 hours you can use quick resume . Or you just don't want to do it in one sitting I can confirm quick resume pauses the timer
