How to unlock the Son of the Tortoise achievement in Total War: THREE KINGDOMS - Definitive Guide
To become emperor as Cao Cao's faction, you have to reach the rank of king. You do not have to rank up immediately when reaching the required amount of amount of prestige. So if Cao Cao is still alive when you do so, you can close the prompt and kill him off first. If you haven't already, you should also unlock Huangdi at the same time as this achievement.
If old age is taking too long or you have Timeless Characters turned on (I forgot I did), the only way to manually kill him off that I found is to send him in combat. In romance mode (not sure about records mode), some characters have "resilience" where they can survive dying in tactical combat. This is marked by a heart at the bottom of their character cards. If it's green, they'll always survive combat. I believe characters with survive yellow health will also survive the next fatal blow, but red means they will die or have an extremely high chance of permanently dying from the next fatal blow. A character's resilience will drop by one level every time they die, and it'll heal back up after a certain amount of turns.
After they die, characters will usually require a turn to recover before returning to the field (their card will be greyed out with a number representing the number of turns they're out for). If there are no surviving units in the army or if army was chased down (attacked twice in the same turn), the army will also be taken off the field and you'll have to redeploy them when available. Because of this, don't necessarily deploy Cao Cao in an army by himself (if you're attacking, you can withdraw extra units immediately and leave him to his fate), and try to pick battles close to your territory so that in case his army gets undeployed, you can quickly redeploy him and attack before he has a chance to heal.
Of course, if Cao Cao gets captured and executed, you can ignore the above. I haven't found a good way to raise the chances of this happening though, other than removing any ancillaries related to capture chance (you probably should be removing all of his ancillaries in the first place).
Final note, while I can't verify it, I personally found that characters killed in duels tend to get caught or outright killed more often. Take it with as many grains of salt as you need, but if you want to try it, try to make Cao Cao as weak as possible as well as compare character stats and health, as the AI won't duel characters they think will beat them.
