How to unlock the Ailment Accumulator achievement in Total War: Warhammer III - Definitive Guide
This is not total collected but total accumulated, here are a few tips if you are trying to be efficient.
-Don't worry about saving your infections during the mid-game, work towards unlocking all the symptoms.
-When you are very strong (3 or 4 provinces well defended and almost fully upgraded), build your cultist camps, use the first option (5 infections per turn).
-Mid- to late-game, stop spending plagues as much as possible, unless you can guarantee they will spread plenty and likely, generating more infections in return.
-Build armies and fight field battles, these can give you over 200 infections. Siege will give you a little over 10 or 30 with a recipe, and only if you sack. Defence battle will give you between 40 or 50 if you fight them manually. I highly recommend to change difficulty to Easy.
*Note: after researching all the Technologies that increase your chances of spreading a plague, you could wait for a rift cycle, use a recipe on Ku'gath, in a province with the Commandment to increase plague duration and further increase plague spreading chance. Then start jumping all around the map spreading plagues (attacking armies or settlements can almost guarantee your plague will spread, although that might not be necessary). I mention this as a last option because fighting field battles is more efficient and you require some luck in order for your Ku'gath plague to spread enough to pay for itself and start generating an income of extra infections.
