This solution is now outdated. You need to hit Y on the main menu before entering the game now, then customize from there.

Both of the solutions here now appear to be wrong.

The ability to customize the character while in the game seems to have disappeared.

Instead, you have to hit cn_Y on the main menu of the game i.e. before you join a game. You can then change any piece of clothing that you want and the achievement should pop.

Once you enter a game, pause with and select “Customize Character” to edit your character. Without any DLC, you should start the game with a number of different shirts, pants, and shoes unlocked. Change any of these and then back out of the pause menu to earn this achievement.

You can do this at any time while you’re in a game session (it can be solo or co-op, local or online). When you’re in the game, press  and go down to “Customize”. Choose any category and change it to anything you want. As soon as you do, the trophy will unlock.
