The game has two difficulty selectors when you start a new game, puzzle and shooter. Set both to Hard and prepare to rage!
This game mostly requires two playthroughs as you will likely want to do the medal achievements and the level specific ones on easy. It is also much easier to complete the game with no life upgrades on easy instead of hard.
My recommendation is to complete the game on hard separately from the others, Good Luck!
