This is a story line achievement for completing Chapter 5. This game can be a bit confusing on the Chapters if you don't go into replay level where they are listed.

Each Chapter consists of 5 Samples so Chapter 5 is Sample 21 - 25, this achievement could just as easily be labelled "Complete the Game on any difficulty"

I completed the game first on Hard/Hard and it popped no problem after it awarded me my medals. Good Luck everyone!
