just complete the last checkpoint of each decepticon mission (dark energon, fuel of war, iacon destroyed, death of hope, and the final guardian) on medium difficulty settings (you do not have to do the entire mission from the start)

Also the complete on hard and medium achievements are stackable. If you beat it on hard, you automatically get medium (and easy if you didn't beat it on easy at first.)
