This achievement can not be obtained from Chapter 1 because there is a Shrine that you are forced to use near the beginning of Chapter 1 to progress.
I was able to get the achievement from completing Chapter 2 without dying and without using a Shrine.
Play on the easiest (Kabuki) difficulty to make it easier on yourself.
Use the trap in the caves in Chapter 2 to kill the enemies without risking yourself.
Try to use the finishers (X, X, Y to stun, then RB to finisher) on the armoured enemies to regain some health.
Make sure to pick up the health and stamina upgrades as you go through Chapter 1 and 2.
