In order to unlock all abilities on a character, you need to be Class 4 (so all the abilities are available) and level 99 (to have enough skill points to unlock them all). Each character has their own achievement for unlocking Class 4, so I'm not going to go into that here.
I highly, highly recommend that you do this on a New Game Plus playthrough, since you get an ability that gives +300% experience upon starting NG+. You can also choose one of the same characters from your first playthrough and keep their level (their class gets reset, so you will still have to do everything to unlock class 4).
The 300% boost, combined with Lil Cactus and my 60+ levels from the first playthrough meant that I hit level 99 before even unlocking Class 4 and doing zero extra grinding. As soon as I re-unlocked the final class, I dumped all the points and got the achievement.
You could still do this on the first playthrough of you really don't want to touch NG+ but you might go insane from the grind. Your choice.

The +300% experience on NG+ and the occasional triple experience from the Li'l Cactus made leveling extremely fast. You can just rush to the boss. By the time you get to choose you Class 2 I was level 19 in my first playthrough and during NG+ I was level 25 only fighting bosses.

why on my 3rd play through, with Falco, Lis and Kevin , this last one didn't keep his level???

You will need to get to a minimum of Level 95 to unlock this achievement, as well as getting to Class 4 for whichever character will be unlocking this (see Ultimate Guard (15G) for more info on that process). You'll need to put at least 53 points in each of the five areas (strength, spirit, stamina, intelligence, and luck) to unlock all of the abilities. The final 10 points in each area only unlock another stat boost. Getting to Level 99 will allow you to fully max out all five areas.
You should go for this achievement on your first NG+ playthrough, as you'll have a skill you can equip called Love of Mana which gives +300% XP for every single battle. Additionally, if you collected all the cactus collectibles on your first playthrough (or at least 40), you'll still have that +300% XP boost which can proc randomly after battle, and they do stack, so you'll sometimes get +600% for certain battles. This allows you to level up extremely quickly.
