How to unlock the Epilogue achievement in Trials of the Blood Dragon - Definitive Guide

After you have completed all the missions (Final Reckoning will be the final one) and found all five secret keys to unlock the extreme missions, the game will notify you that a new ending is available. Play through Final Reckoning one more time and you'll see a brand new cutscene after the credits (which you can skip).

Thanks for the solution.

Pretty disappointing lack of resolution there. Anyone reading this wondering if they should do the work to see the real ending, don't. Nice rare achievement though.

Refer to "The Truth is Out There" for more information.

The secret ending can be obtained by completing all the secret levels and then completing level #27 'Final Reckoning'. I recommend that you do all the secret levels first, then do the final level as then the secret ending will simply play after the credits (which you can skip). If you haven't completed all 5 secret levels before you do this, you'll have to play through and complete level #27 'Final Reckoning' again.
