This achievement is actually quite easy to get, you will need a little bit of cash.
You need to build a weather station (Approx. $8,000), once built select the station and at the bottom you will see three upgrades available to you, the right one being a 'Storm Detection' system ($7,500) There is also a Tsunami warning system (middle) if you are going for the Head for High Ground achievement too.

Once this is built you will just need a Tornado to hit in order to get the achievement. You have two options here; first is to aim to build this set up on each campaign mission as you play through, you will get one hit eventually. Alternatively you can use sandbox game mode and set the number of events to the highest possible if you want to speed up your chances. (Don't use GOD mode, I have heard this will stop any achievements unlocking.)

So, I had a tornado hit during a campaign mission with the warning system installed and the tornado still killed civilians... is this a hit or miss type of thing?

As far as I was aware it was sure thing. Potentially I can see that perhaps shacks might still be vulnerable? I'm pretty sure I had some shacks and and no casualties though. You definitely had the right detection system upgraded?

You don't need a fully upgraded weather station. Just the upgrade that detects Tornados. It will prevent deaths during randomly occurring tornadoes. Then just wait for a tornado.
