A minister's gaffe is a random event in the game once you have built a Ministry and have hired at least one Minister.
When the event occurs, you can either keep your minister and accept some sort of penalty, or fire the minister. Do the latter, and this achievement will pop.
I had about 5-6 of these in my first scenario.

I played a couple missions and it never happened. I played Sandbox and it happened REALLY quickly. The first thing I did was build an ministry. And I had one free choice, Within like ten minutes he fucked something up and I could fire him. That is after 8 hours of the campaign and it never happened.

A gaffe will randomly occur if you have ministers, so you should always employ some. Penultimo will inform you when one happens, then just fire the minister.
