In order to achieve this you need to abandon two ultimatums at the same time from two different factions.
The easiest way to do this is start a sandbox game in the modern era with unlimited money, no other settings impact this.
The quickest way to get factions to give you ultimatums is to build a lot of buildings they dislike to lower their support for you enough to trigger the ultimatum requests.
I chose the environmentalists and the militarists. If you choose the militarists you can also earn the ‘One Does Not Simply Stage A Coup’ achievement for surviving a military coup. If you are going to choose the militarists, purchase the second set of guards at your palace, they will help you survive the coup. No other military is needed.
The environmentalists dislike Teamster offices, the militarists dislike Embassy’s. Buy and quick build around 16 of each building and you will get the ultimatum tasks to appear.
***You NEED to go into the task lists and click to abandon them both, one after another for this to work. You can access the task list by pressing the back button on the controller (the button with two squares on it)***

The environmentalists dislike Teamster Offices did not work for me.
Instead build power plants that emit pollution + meat/cattle farms. Around 16 each and you'll get a notification on your task list form the envrionmentalists.
ABANDON both (military + environmentalists) of them at the same time and it should pop!

The environmentalists dislike Teamster Offices did WORK for me, just build more of that (like 20). But you don't have to use the unlimited money, cause it disable the achievement earn.
