Available in the Forest Fortress / East Vault area, and requires you to have unlocked the dash ability through natural progression of the story.
Teleport to the Interior of the Eastern Vault, and head down into the large open area by the gate. Here, notice a ghost (if at night time) to the right. Head through there towards the Hero's Grave. At the bottom of the steps, teleport dash to the left of the stairs, across the water, and into a cave. Destroy all 4 leaf piles to reveal the chest.
