Video is helpful. Won't knock you since it's not yours. The door code for 44-45 is incorrect. It should be down, left, up, right, up, left. Probably just an error when they added the text as what is displayed is the code for 46-47.

I've collected all pages and even got the true ending, but no achievement. Anyone else have this issue and know how to fix?

There are plenty of videos on where all the pages are, but I wanted to make a note here that if you are going for this achievement, wait until after you get the regular ending first and then choose Retry and collect the remaining page(s) to get this achievement. If you do this before Game Over, you will have to play the entire game again to get that achievement. There are plenty of pages out in the open that you can see but can't get until later. Just make sure you leave at least one page for later and go for the boss fight.

Just a tip for anyone struggling with putting in the code in the mountain door, It could be that you are making mistakes or i could be that the inputs are timed. I failed 10 times or more as I was trying to be careful. There is an hourglass that slows down time at the most of magic on the beach, I got it first try after using it.

There are 28 pages to collect to complete the Manual (each page you collect adds two pages to the manual itself). You'll find the vast majority of them through natural progression and exploration, as most of them aren't hidden, and some just require you to come back to once you have different abilities. There are only a handful that are actually hidden and you almost assuredly won't find without a guide. Grab any pages you come across as you progress, of course, but don't worry about mopping up this achievement until the end of the game, once you have the ability to ghost dash and restore your physical form. That's when you'll be able to access them all, and mop-up will be quick and easy.

Use the teleporter right at the start of the game and you will be trasported to the hub-ish area and the pages are just north of the pad.

Just south of the entrance to the Dark Tomb these pages are on a block, you will need to teleport dash to get them.

After activating the 2nd pillar in the Forest Fortress, these pages will be on the path.

Once you get the teleport dash from the Cathedral and leave the Swamp back to the Overworld these pages are on top of a pillar to the left.
