How to unlock the What just happened? achievement in TUNIC - Definitive Guide
After opening the large golden door near the start of the game - the first time you die from an enemy you will be transported back to the golden door room, after you walk towards the blue fox it will revive you and you will earn the achievement. If you die before opening the door, I believe you will respawn on the beach and won't get the achievement.
NOTE:
If you've been playing with "No fail mode" enabled and have already used all 3 colour keys to free "The Heir", then the initial section where you get revived won't play as The Heir won't be in the prison anymore so be sure to die before then, to ensure you don't need to start a new playthrough. (This won't be too much of an issue as pressing "New game" won't override your previous playthrough)
