The Mantis boss is located at the end of level 5: The Catacombs. You will enter a portal then approach a large Mantis statue with a key nearby luring you in. Obviously, the statue will come to life and attack.
The Mantis jumps around the area, clinging on walls and the ceiling as well as the floor. Just back away and keep firing with whatever you've got. As the Mantis takes damage it's movements become more erratic and it will even start destroying the environment. There's ammo around so restock as you need to.
The achievement unlocks once you slay the beast!

You will obtain this achievement at the end of the Level 5, 'The Catacombs'.
Your best option is to probably use the particle accelerator/shockwave weapon at full charge to quickly deplete the Mantis's health, ammo will spawn consistently around the outside walls but overall it should not be a hard battle.
