There appears to be 5 Illnesses that make up this achievement. I was stuck at 75% for awhile before I got a bit of help figuring out what to double check.
Square Eyes- Comes from a computer lab (can get anywhere)
Blue Blood- Common in Poshos (a good course for this is Money Wangling as all entrants are Poshos).
Eye Candy- Common in Gastronomy
Cauliflower Ears- Get from Cheeseball injuries.
Broken Leg- Musicality Career track.
I believe that there are only 5 here but it’s odd the way it implemented for me. If anyone has any additional illnesses that I missed, please let me know in the comments.

I've tried to get all described illnesses in a single campus with all needed courses. I've observed students from specific courses and cured all illnesses mentioned above, it didn't work.

With his solution I indeed managed to get achievement after a bit more that a year:

"if anyone needs help: I got the achievement within 20 minutes when I set up a campus in sandbox mode. Make a campus and have four level 5 courses: academic exercise, internet history, gastronomy, and funny business. Build all of the requirements for it, and then hire a lot of assistants and train them in medical care. I built three pastoral rooms and four medical care rooms and just had the game auto play for around one year before I got the achievement. It took more time setting it up than it did to get it."

How did you do Eye Candy? I can't for the life of me do it, even on a sandbox version with just Gastronomy none of them get it?

I observed the achievement progress in 4 steps: 25%, 50%, 75% and unlock. I believe only 4 illnesses are required.
I got the first three on a campus with Academic Exercise + Internet History and the last with Gastronomy + Funny Business.

I assume the illnesses are: Square eyes, Eye Candy, Califlower Ears and maybe Blue blood.
Broken leg is not required.

Just popped for me after curing Cauliflower Ears in Fluffborough (the Cheeseball level). I'm in the post-game, but I completely forgot to build them a Medical Office, despite training all my Assistants in First Aid. 🤦🏻‍♂️

Hmmmm my progress bar moved after curing "brain freeze" which is common in swots. That moved me to 50% having cured two.

Previous condition I had cured was Cauliflower Ears

I agree, it seems to move in 25% increments so that would mean 4 illnesses need to be cured.

I was aiming for this achievement specifically, so I know "Square Eyes" and "Eye Candy" can be used for this. Unfortunately I wasn't paying attention so I'm not sure what the first 2 were.
