I got this on Lower Bullocks. Its straight forward to do.
Firstly turn off auto saving.
Manually save and have the timer on pause.
Then start selling items in the hospital (be careful not to go too fast as my game kept freezing after i sold too much or too quickly)
You will start to see the critters appearing on the floor. They usually are under vending machines and fire extinguishers. Once you can see 10 you can begin.
You cannot shoot them while in pause so hover over them then quickly press then

Repeat until you kill 10.
You will know if you have not missed one because you will see a kill counter icon appear.
Once done you can load up save a carry on playing.

seems to be glitched or something, i got my achievement at 9

You don't need sell just move it they hide under vending machines arcade machines and reception desk I just move one while paused got ready with aim un pause and steadily picked of odd ones re paused and repeat you do need 10 but it unlocks after 9 streak but it doesn't include 1st kill?

The game has since been patched; pausing the game restarts your kill streak. (This means lining up your shots won't count).
The only way I managed this was to quick save after each successful kill and then quick load when I didn't make the next one.
Notes: Do not pause the game when going to the save/load menu, and the kills do not stack on slow time (only normal and fast).

Be sure to use quick save/quick load. You pick up an object, a “rat” runs, pause, save, release the pause and shoot. If you miss, then load.
