This is very easy to do. Set up a de lux clinic on the level under look hotel and make sure that you have purchased the gold star award with kudosh in the store. Fill the room with them until it says level 5 on the top of the screen and press y and it will pop.

In the first level, named "Underlook Hotel", you'll need to build a level 5 De-Lux clinic. The bigger the room, the higher the prestige, but you can obtain this using a 3x3 room if you're short on space.
Before you build the room, make sure you've unlocked/purchased the Gold Star Award. If not, you can purchase it via the Items panel for 200 Kudosh. It'll cost $50 to place each time, but it will add extra prestige to your rooms which will greatly benefit you in this instance.
Decide which size room you'd like, then add the necessary machinery/furniture. When you're happy, spam the walls with the aforementioned Gold Star Award until the room hits level 5. Now, build the room and the achievement will unlock.

This achievement is obtained by having a De-Lux Clinic at Prestige level 5. You can do this simply by filling the walls with Gold Medals.
To get 3 stars you'll need to raise staff morale to 80%, so it's good to do this to all of your rooms when money is less of a problem anyway.
