While in any campaign hospital.. On the objectives screen in the top right, move over to the middle section for online. Then click a challenge. I chose cures and it will then automatically select friends from your xbox account to race agaisnt to get the most cures. Your friends don't need to play or even own the game. Just wait the days for the change to complete.

the button is grey for me,any help?

The button remains grey in the first level. You need to advance to later levels.
