Making a permanent potion is easy, but the game doesn't explain how to do it very well.
What you need to do is add one permanent ingredient (which are labelled as such in your inventory and are easy to find throughout the game) and a mana or health potion; it doesn't matter which.
To add an item to the mixing pot, just press A when the cursor is over it.
Once you have a health or mana potion and your permanent ingredient in the pot, mix the items (click on the pot), then save the result; you don't have to give the new potion a name if you don't want to. Then click on the potion. The achievement will unlock as soon as you've done this.
Note: the higher your alchemy skill is, the better results you'll get from the permanent potions you make.

For this you will need two or more ingredients with effects that are permanent. Both (or all) must be permanent or it won't create the right type of potion.
