To get this achievement, you must first unlock the following (the four elements can be done in any order, but this order is preferable):
The Found the Relic Frame achievement in Two Worlds worth 30 pointsFind the Relic Frame in the main storyline.
The Found the Earth Element achievement in Two Worlds worth 31 pointsFind the Earth Element in the main storyline.
The Found the Water Element achievement in Two Worlds worth 32 pointsFind the Water Element in the main storyline.
The Found the Air Element achievement in Two Worlds worth 32 pointsFind the Air Element in the main storyline.
The Found the Fire Element achievement in Two Worlds worth 33 pointsFind the Fire Element in the main storyline.
Once you have those five achievements, head to Qudinar. If you forgot the way there, the in-game map will help! Achievement will unlock just outside of the city with a 'cutscene'.

This map shows the location of Quidnaar (marked as #9)
and the area which you need to turn the quest in (marked as #8)

Just to clarify, you have to walk to the area just outside of the city in Quidnaar (right near the bridge) -- talk to the NPC Gandohar, who is wearing a red robe/armor near the wooden structures. This is right at the fork in the road on the mini map.
