Two Worlds

The simplest way to get this is to enter and exit any underground location you find (dungeons, caves, anything that goes below ground) even if you don't intend to explore it. It's also easy to do this with maps online, and if you're trying to find all undergrounds—and this achievement on the way—it's best to start a fresh game where you don't have any of the undergrounds discovered but accidentally unvisited.
