How to unlock the Kay. Tee. Eff. Oh! achievement in UFC Undisputed 2010 - Definitive Guide
If you have a TURBO controller follow these steps:
1 - Beginner difficulty, Heavyweight, choose Lesnar and Sanchez as your opponent.
2- Set the turbo controller to keep pressing A and X and holding the pad toward the opponent.
Following these steps your fighter (lesnar) will punch and kick Sanchez until the end of the fight and after that the controller will rematch automatically.
(I would suggest to do this and then sleep or do something else, I got this achievement within 6 hours after setting this up).
PS: Sorry for my english, I'm brazilian.

I have this running now for 10 plus hours and nothing yet has anyone else been able to unlock it?

Here is the video replay of when i unlocked KTEO. This achievement is so random and when you see my replay you'll be like "This is glitched, i've done this a 1000 times" I'm writing this guide just to explain what i did exactly in my fight and what i think you can and can't do in able to unlock this pain in the ass achievement.
Most of my pointers are very simular or the same as CoMANdo DOOM;
1) This achievement can be unlocked on beginner difficulty.
2) This achievement must be against a UFC roster fighter and not a CAF. I had over 600 knockouts and the majority was against CAF's.
3) You can use your CAF to obtain this achievement in either career, exhibition or title defence mode.
4) There's no time limit to get this as i unlocked my achievement 1:40 0f round 1. People have also achieved this in round 2 and 3.
5) I'm pretty sure damage does not count against this achievement as my opponent had taken some big punches and elbows and was pretty bloody.
6) This achievement can be unlocked without the use of swaying. Counters are your best bet for the flash KO.
7) You do not need to reset the system cache and play this offline. I did however after 600+ knockouts then played a heap of matches offline with no joy then signed back into live an updated the game then somewhere down the line got this online.

Ok, as these guys say over at x360a i used Brock vs Sanchez at the tryout gym (AT FIRST), ran over hit with elbows, backed off then repeated until the opponent was knocked out. I tried this method for 50 fights with no luck what so ever so i was about to give up then selected different fighters. For my fighter i clicked on random and for opponent i selected Pat Barry. My random turned out to be Justin Mccully so i used the same strategy of running over from the bell and unleashed big punches and elbows. I then backed off and ran in again throwing just big punches and elbows..... still no KO or rocked opponent. I backed off for a 2nd time then went in again and managed to get caught with an elbow but immediately threw an elbow of my own to counter and achievement unlocked. I didn't expect this to unlock basically due to the amount of punches and elbows this guy took. The difference between using Brock and Mccully is basically the power difference. When using Brock 4 big hits would rock your opponent where as using a weaker fighter gives you more chance to get a flash ko.
I hope this guide is of some assistance to you all as this achievement is not as easy as just do this and that and ping.
People should not vote negative on these guides just because they have not got it by following them to the letter as it just dont work like that. What will work for 1 will not always work for another.

I hope it helps ya dude. Check out the link for x360a. You'll see a load of peeps got it this way. I know the replay doesn't really help an awful lot but if you watch it to the end you'll see me backing off then moving in.

I've tried all the other solutions with plenty of Flash KO's and no achievement so now I'm on to this one and I hope it works.

Okay. I got this achievement a while back and ever since I've been getting messages on how I got it. This is exactly what I did. I used Shane Carwin vs Mark Coleman. The difficulty was Expert. I walked across the ring and when he tried to touch gloves I jabbed him in the face 12 times. Then I through a powerful right punch and Knocked him the F**K OUT! I'm not really sure but what I believe the requirements for this achievement are: You have to knock him out in 30 seconds or less, You can't rock him (black and white screen) and he has to be out cold and unable to stand next to you getting your hand raised. There will also be a little cutscene of the doctors around him. Once again I'm not sure if that's right but nothing else worked. Oh and you CANNOT do this with a CREATED FIGHTER! The updates got rid of that.

fact of the matter is its just a super huge pain in the ass and isnt glitched
how i got lucky was
Lyoto Machida VS. Rich Franklin
i threw a couple counters then i got lucky enough that when he went to go throw a right kick to the body i had hit right head kick at the same time so his head just happened to hit right into my head kick
this achievement isnt all luck its all about timing it out cause right after i got it i started another profile and as shit would have it i got it again do the same thing
just watch the oppenents head if you can throw a kick as there moving there head the same direction the cheevo is yours
not much help i know but there is really know way to guide someone to this one

Ok I finally just got this after countless hours of trying (Over 700 matches just focused on this achievement alone). Easily one of the most frustrating cheevos I have experienced. After trying the matchups I have seen others have success with (Lesnar vs Sanchez, McCully vs Barry, Hendo vs McFredries) I was ready to give up.
What I did was use Rampage (Red corner human) vs Krzysztof(Blue corner computer)
-3 rounds, Beginner difficulty, Tryout arena, Random ref.
-Played with normal updates/dlc and connected to xbox live
All I did the whole fight was slip/sway from side to side and throw hooks trying to anticipate when he was going to punch. I would try to sway out of the way of his punch and throw one of my own from the swayed position .When I got the KO he was throwing a right uppercut and i had swayed to my left and thrown a hook.
It came at 1:26 of round one and had landed about 10-15 head shots before this one. Another thing to note is he didnt get up for the winner announcement.
Obviously others have had the elbows method or the first shot KO method work for them. I tried those to exhaustion and couldn't get it but that's not to say they aren't right. This is just another option for those struggling.

Here is a cool trick I discovered to basically get a Flash KO every time and hurt the fighter so bad that he can't stand for the official announcement to go along with some of the previous solutions posted.
SHORT EXPLANATION:
Basically what you do is get your opponent into the rubber guard and land short punches with your free hand their face will get cut and bruised badly the more you do it then all you have to do is touch them with a few strong strikes on the same side as the damage and this should greatly increase your odds of getting the achievement.
DETAILED EXPLANATION:
1) Use a fighter that has the Rubber Guard technique, usually Jiu-Jitsu fighters have it (i.e. BJ Penn, Shogun Rua, Frank Mir, etc.) although BJ Penn would probably be the best bet for doing this.
2) To gain the Rubber guard position you must first be in Open Guard down (Opponent must be in your guard) best way to do this is to pull guard. Initiate a Clinch by moving the Right Stick towards your opponent. Depending on the fighter you can pull guard from multiple clinch positions but usually any Jiu-Jitsu fighter can pull guard when they have Double Underhooks. You can maneuver from one clinch position to the next by Rotating the Right Analog Stick in any of the following directions (mixing them up helps) Down to Left, Left to Up, Up to Right, Right to Down (6o'clock-9o'clock, 9-12, 12-3, 3-6). Once you are in the desired clinch position you pull guard you hold LB and rotate the Right Analog Stick from Down to Left (6o'clock to 9o'clcok) may take a try or two if the opponent blocks the attempt.
3) Once you are on your back with the opponent in your guard while holding LB move the Right Analog Stick Down then roll it to the Left or Right (6 o'clock to 9 or 3 o'clock) keep performing the motion until you gain the position. Your fighter will grab his own opposite foot (grabs Left Foot with Right Hand or Right Foot with Left Hand)
4) Once you have the opponent in Rubber Guard continuously land strikes with whichever hand is free by pressing X or Y. If the opponent breaks your Rubber Guard just put them back into it by repeating step 3. If the ref stands you back just repeat from Step 1. The more times you do this the merrier, but usually 2 or 3 should be enough. Eventually you will notice your opponent gets cut and bruised very badly. 4.Began Title Defense Mode with Brock Lesnar
5.Rushed the opponents with left and right punches (++/), won 3 fights with KO's (the 2nd was with Frank Mir)
6.The 4th fight was again with Frank Mir. Just rushed him with punches as done in previous fights. Rocked him once, waited he stood up, continued to punch him - BOOOM! - achievement popped up. The resulting damage was: head - 68%, body - 3%.

I just got this achievement and I feel like a little kid back in the nineties that got their first Tickle Me Elmo!
Here's to squashing the "facts" that some people THINK they know.
1. It DOES NOT have to be in the first minute! I got it around the three minute mark.
2. You DO NOT have to sway. I used a COMBINATION of sway counters and strong punches leading up to it. (Holding forward not LB). I finished him off with three elbows in tight.
3. It DOES NOT have to be on EXPERT. I did it on ADVANCED.
4. It DOES NOT have to be a single punch. Once again, I threw three right elbows and he tried countering in between the second and third. Obviously wasn't smart on his part.
5. He WAS cut up a bit. I really don't think you have to worry about damage too much.
Here's exactly what I did.
I went and deleted all of my UFC saves. Afterwards I went and cleared my cache. I don't know if either of these things worked but one or both must have done something cause I got it on my fourth attempt. It didn't take long considering I had about 600 total matches before hand.
I played the game offline so it couldn't update. I know some people got their achievement online but I didn't know if the patch had anything to do with the rarity of this achievement so I figured better safe than....not safe.
To start off I picked Dan Henderson and I fought Drew McFedries. Now in all of these fights I fought with an aggressive/countering style. Meaning if he struck I sway countered and if he backed away I chased him using the power strikes. (Once again not holding LB but holding forward.) During my final attempt he started backing away so I lunged in at him, hit his body once and threw three right elbows up close, knocking him out with the last. HE DIDN'T GET ROCKED AT ALL DURING THE FIGHT!
I seriously thought I was dreaming when it popped up.
SYMBIOTEHELL -"I managed to get mine in Career mode on Beginner. My character had 52 strength. Ran across to the opponent and throw a hook LB & X and it caught him. Knocked him out in 0:01."
As you can see, this once again disproves some people's theories about this achievement.
I mean, I still don't know the exact requirements for it BUT this is how I did it.
I hope this can help you guys because I certainly know that it is an unnecessarily frustrating achievement.

Just got this even after rocking him and knocking him down once then i let him get up and as soon as he got up i hit him with a spinning back hand level 3 of my CAF against silva in the title defense mode on beginner ! Just the next fight i was able to do exactly the same thing. Guy never got up.
This may not be the best solution, but, this is how I got it:

(Need to get my Xbox back to an internet connection to reflect me getting it) I was going through Title Defense mode with Brock Lesnar, and put down Fabricio Werdum. I was just punching away, trying to knock him out quick. I rocked him once, he recovered, and I went on a punching spree once more. He was knocked out, and once he hit the canvas, the achievement popped up. The doctors were surrounding him, match over, and when I looked at the body chart, he had 53% head damage. So, your best bet is to go through Title Defense mode with Brock Lesnar, and hope you get Werdum in the D Class bracket like I did.
