Progress through the game until you get access to unlock the anvil located just above Death in the Necropolis.
Once you have this unlocked just combine any 3 items to craft a new item.
In order to get the 3 affixes that you need the bottom item needs to be a purple quality item of any type. This determines how many affixes you will get when you craft. It will tell you below the name of the item you are crafting if it has 3 affixes and it will also be a purple item.
