Upon immediately piloting the mini-sub Moon at the start of the game, I accelerated out of the rocky tunnel and headed straight up for 5 seconds until I broke the surface.
Achievement immediately unlocked.

Also, this can only be done in the minisub. It won't let you swim to break the surface.
