You must first complete

The A Friend In Need achievement in UnderMine worth 16 pointsAssist Beltrame

.

After Beltrame talks to you in the dungeon the last time, the room to the right of the main starting hub will open for potion creation and purchasing. There's an "Empty Bottle" item being sold for 2,000 gold. Purchasing this unlocks the 2nd slot for potions, and the achievement.

Are you sure? I thought so too.. I purchased that and did not get the achievement.

I bought the extra slot and it hasn’t popped for me. Maybe glitched.

I think most of achievement seems bugged, i've unlocked the achievement for buy all upgrade with buying the first upgrade ... the heal 1000 hp are glitched at 96% for me, same for the spend 10000 gold, the 50000 gold as unlock normaly ... hope the game have an update for this

Definitely bugged. I popped the Bandolier achievement when I bought the second upgrade. Playing on PC.

As far as I know that's the only requirement. It popped literally 2 seconds after I bought the Empty Bottle, and I didn't have any other potions in my inventory, so I don't think you have to have the slots filled to count.
