How to unlock the Hexcellent achievement in UnderMine - Definitive Guide
After defeating Seer (the final boss of the mine) for the first time, you'll unlock Valeen as an NPC in The Hub who will offer a number of hexes to make your runs more difficult. The easiest combination of hexes for this achievement are Chaotic Offering (all items hidden) and Dreadful Fog (enemy damage and health doubled.)
The first hex can be countered simply by having a lot of gold and being able to buy everything from all shops without consequence - doing a regular run or two should get you more than enough gold for this, and you probably shouldn't attempt this until regular runs of the mines are trivial and you have most of the blacksmith upgrades. Beyond that, it's a matter of luck in getting good relics that synergize with your desired build and knowing the approximate value of various items from doing repeated runs. You can bring in starting items from Dibble's shop, a relic from the blacksmith, and/or a potion from the alchemist to make your run easier.
Conquering the second hex just comes from experience with the bosses and enemies. One major note is if you don't like the bosses, you can defeat all of them on a regular run and then just quit out before completing it and restart the run with the hexes active and still get the achievement.
Make sure you change your summoning stone level down to 0 in the pause menu before beginning your run. Even in the best of circumstances, sometimes you'll get a room that is stacked against you and really wears down your health. In these instances always remember that quitting an option - if you feel like you've taken too much damage in a room you can always pause and quit game (or alt+F4 if you're on PC.) As long as you do so before you hit 0 HP you'll respawn at the beginning of the floor and have a chance to try again. The autosave seems to trigger as soon as you die though, so make sure to quit before then.

I tried the hidden items one plus tougher enemies and I just couldn't do it, I like to transmute too much, also it was annoying picking up talismen when I didn't have curses.
Instead I went with red hot nuggets (gold damages you) and tougher enemies, I also crafted the pillow to bring along to negate some of the extra damage. Also I've been using the blessing familiar, it's pretty great when you can get 50+ blessings in a run. Having the altar in a bottle potion helps.
Edit: obviously you want to bring a lot of gold in with you, I had around 200k and ended with around 120k. Also when opening chests I would just exit the room and re-enter and the gold disappears. ALSO I didn't make this mistake but you definitely don't want to open a boss chest since that gold is automatically absorbed.

Great guide. One thing to note is you don't have to beat any bosses just get to the final room with two hexes. I just started a run, skipped to Golden Core with the fast travel and then rushed the ending. Achievement pops as you enter the room after Seer. If you do it this way then a better second hex is to remove relic rooms as you wont need them!

After beating the game, you will be able to assign yourself evil eyes from your grandmother in the main hub, which can make the game quite difficult. For me, the easiest ones turned out to be a combination of the evil eye for the opposite effect of altars with prayers (in fact, at the altars you can only remove curses) with the evil eye for becoming a target for enemies (I didn’t notice the difference).

killing 4 bosses on the way to the lizard is OPTIONAL.
that is:
1) take the evil eye "after killing the boss, relics disappear" "altars work the other way around"
2) run through all the floors ignoring the bosses (immediately jump into the already open hatches into the next biome)
3) kill a lizard and get a trophy.
