Probably hardest achievement in the game. Levels with horseshoes have horseshoe logo on level select. If it's black&white, it's not collected. If it's colored, it has been collected. Just go in to those levels that are missing and collect them. Here's full video walkthrough that has all horseshoes & achievements.

Worth mentioning that you can collect the horseshoe and just quit/skip the level and it will stay collected.

This was the only achievement I had left as well.
Worth mentioning that its easy to see which Horseshoes you are missing, as when you go to chapter select, the ones that are shimmering indicate you have already collected them.
So just look for the levels where the Horseshoe symbol is plain white and not shimmering.
