Earliest chance I found for this was Chapter 3.
You'll come across a room filled with chemistry equipment that you can break open to get some ingredients for Chloroform. Mixing Chloro + Form will get you that, but mixing Water and Sodium will create an explosion that instantly kills you.

You can die while tampering with your inventory as soon as chapter 2 by combining Gasoline with a Cigarette Butt, but you won't unlock the achievement via that method.
This achievement can be unlocked in the same place you find this medal (collateral damage) in chapter 3. A room with chemical elements such as Sodium, Water, Chloro, Form....
By combining Sodium with Water, you create an explosion that instantly kills you and the achievement will be unlocked. Be sure to save your game beforehand.

You can unlock this as early as the middle of Chapter 2. In Chapter 1, if you search knocked out guards you should be able to locate Cigarettes and a Lighter. The Lighter is on the guard one screen north of the guard with the eyepatch, the Cigarettes are on one of the two guards in the screen east of the screen where a prisoner asks you for one. Once you have both, go back to said prisoner and give him the Cigarettes and then the Lighter, which gets you the Cigarette Butt, one half of what we need.
Later on in early Chapter 2, when searching the first warehouse for the Raw Meat, several boxes in the same area will have Gasoline in them. Once you get one all you have to do is combine the Cigarette Butt with the Gasoline and boom, Achievement Unlocked. If you missed the Cigarette Butt but got the Lighter that might also work, but I have not tested that.
Later on in early Chapter 2, when searching the first warehouse for the Raw Meat, several boxes in the same area will have Gasoline in them. Once you get one all you have to do is combine the Cigarette Butt with the Gasoline and boom, Achievement Unlocked. If you missed the Cigarette Butt but got the Lighter that might also work, but I have not tested that.
