This achievement requires that you complete all 20 bonus levels in a single session and in under 45 minutes. The timer will start when you enter the portal for the first bonus level and ends when you complete the 20th bonus level.
You can play through the levels in any order but I found it best to start at the top of the lighthouse and work your way down as levels 16 and 13 were the most difficult in my opinion.
The 45 minute requirement is pretty lenient. 40 minutes is definitely possible and 35 minutes might be possible. You can die a lot and make a lot of mistakes and still get this done in 45 minutes.
I suggest playing through all of the levels several times to learn the specifics and particulars. Here is a playlist with individual videos showing what I felt were efficient routes through each of the levels.

This was honestly my first attempt at a real run after having figured out and practiced the levels individually over a few days. It's sort of a mess as I forgot some of the puzzle specifics on a few levels and lost a lot of time while floundering to remember how to get through. I also died a bunch although I did pretty well on what I feel are the hardest levels.

I didn't use the Xbox button. I only did practice and practice only the challenges hardest. In my opinion the hardest levels were lvl 16 and lvl 18. I used the slow cam in these levels and I got my achievement with a time of 42:00 min. I used a phone with cronometer, a towel for my sweaty hands and somes videos with puzzles that I forgot. Just practice on every bonus level and when ready start your adventure, when u want to restart close the game and relaunch it. U have time to make a lot of mistakes and no matter in wich sequence u complete the Bonus level. In my video i started from bonus level 5 cause when i relaunched the game i found myself outside that level and the hardest challenge for me was level 16, i wasted around 5 min in the video on that challenge but got the achievement anyway.
For those who like me didn't find all answers to essential questions:
1) absolutely all time count once you start: this includes play time, death, and all loading times...
2) quit a level doesn't not restart the countdown (I did it once)
3) re-enter a level already done doesn't affect the trophy, and doesn't reset the countdown (I did it once)
3) you can die (as many times you want ) this doesn't affect the trophy (I died several times)
4) I strongly guess countdown is set (or reset) when you start from the main menu's game (where you can choose your save slot); I re-start from there a couple of time when i was not satisfied with my start, each new start with a countdown running on my phone and I get the trophy.
5) Trophy unlock after the last loading time of the last achieved level to return to the hub.
6) Order of the levels doesn't matter: I made it from 20 to 1, without any respect of any order.
7) You can use (and abuse) slow motion, this doesn't affect the trophy (I use it on level 16 & 20), however it takes slightly more time to achieve levels
8) I would recommend to do all levels of a section before trying another section, as travel between each section takes time, and coutdown is always running... So choose to start with the section with the most struggling level for you...

This is the make or break trophy for this game. You have to complete all 20 bonus levels in less than 45 minutes. Keep in mind that the time spent in the lighthouse and the loading screen is also counting towards the 45 minutes! To clarify, you are allowed to die or restart levels as often if you want to. I think the order of the bonus levels should not matter but this might need confirmation.

All levels have to be completed in one successful chain. If you’ve made a real doozy of a level you can back out to the main screen and re-load your save file. You’ll have to complete all 20 levels again.
I personally completed “motion sickness” first it’s a long level if you mess up at the end then you can re-boot your save file. “Tilted” is a hard level also within the same bonus area and is good to get out of the way. I agree with all of the above, but for my part I will add that you can take the tests in any order convenient for you. Personally, I started with “fire” tests (at the factory), because... They were the most difficult for me, and then in descending order of difficulty.

Pausing in the Lighthouse and levels stops the timer. If you need to refer to the video or need a break, then a pause will not spoil the trophy (checked personally).
