How to unlock the No stone unturned achievement in Unravel - Definitive Guide

The game has total of 55 secrets / collectibles in 11 chapters. Chapter 11 has no collectibles. All collectibles are immediately saved when picked up.

In the main menu you can see on any chapter picture how many and which collectible you have collected in the chapter. The order is from left to right, so if you have found, for instance, secrets 1, 3, 4 and 5 you can see that you have missed the second secret on that chapter because there is a an empty space between the first and third button.

Chapter 12 has no picture (it is a flowerpot) but you can see the secrets on the chapter 11 picture.

You can find timelines for each chapter in the spoilers. The last spoiler contains the video again.

Have gone back and got the remaining secrets, however nothing has popped. Anything else I need to do?

Give up. This achievement is glitched and nobody has it.

This is a confirmed unobtainable achievement for anyone who reached 100% on this achievement progress and remains locked.

Have the same issue, collected all (5 on each level) and it is stuck on 98%.

Yep glitched for me too, 81%. Also, The end is not the end achievement didn't unlock. Never would have started this game had I known.

After a hard reset the tracker says 100%. Just a matter of time before it unlocks so.

Glitched for me too, but popped up after a while. What I did was:
1. Closed a game completely and run it again.
2. Logged out and logged back in the profile while I was in the game.
3. Entered and exit every chapter (without completing of course).
4. Gave up... and then suddenly while doing another achievement with mosquitos the "No stone unturned" popped up. Maybe it needs some time of gameplay.

Glitched for me, stuck at 96%. I checked literally every single level, and they all say « 5 out of 5 secrets found » but It won’t unlock. I tried what Doman said, I even did a hard reset on my console, but nothing.

The basics: There are 11 levels in the game plus an epilogue. The first 10 levels and the epilogue each have 5 secrets as collectibles (none in "Last Leaf"). This makes a total of 55 secrets for this achievement.

Each picture frame in the hub will show you how many you have found for each level and their order in that level (for example if you have the 3rd and 5th sequentially, it will show the markers at the bottom middle and bottom right of the frame with gaps at positions 1, 2, and 4.). This is very helpful for knowing exactly what you're missing. The frame for "Last Leaf" shows the secrets for the epilogue section after you're out of the snow.

There are a total of 55 secrets in first 10 chapters and Chapter 12. Chapter 11 "Last Leaf" does not have any secrets. Once a collectible/secret is picked up it will automatically be saved, so you don’t have to worry about losing that progress. You can tell what secrets you have for each chapter by looking at the picture frame for a specific chapter. There will be little markers on the bottom of the frames denoting each secret found, and they are listed in order.

360GameTV on YouTube has a great video guide for finding all 55 secrets, without commentary. I’d recommend using this video if you need help finding some secrets since some are slightly difficult to get on your own.

If you missed an item, then when you subsequently pass the level and find the missed secret, you do not have to play to the end of the level, just go to the checkpoint (a skein of thread on a nail) and you can safely exit the game.

When you find a secret, it is not necessary to complete the level to the checkpoint; you can immediately go to the menu.
