Insane difficulty really is insane! I recommend you play through the Campaign on Casual until you hit the last level of the chapter. Switch the difficulty to Insane and complete the level to get the achievement. Here is the list of levels I chose to complete Chapter II. Where I have written [Select], I have been given a choice and this is the level I selected.

Important: If you select different levels to the ones I have listed below, your last level may also be different

(03) Sinkhole
(04) Market District
(05) Tank Crossing
(06) Kargo
(07) [Select] Torlan Delta
(08) Hydrosis
(09) [Select] Serenity
(10) Suspense
(11) Downtown

(13) Kargo

Insane
(14) Serenity
Card: Iron Guard Reinforcements - Gain 2 Iron Guard team members

Play card "Iron Guard Reinforcements" to even the numbers (you'll need it!). First head for the Mine Node and hold it down until a Leviathan is built.
Jump in and drive, park the Leviathan between your Prime and the enemy's. Press A to plant it, zoom in with the center gun and aim for the top of any blue nodes (yours / enemy's prime). Just hold your position and watch your bot rush with the orb. Keep firing shots to kill any enemy units. Once you hold both primes, jump into a fast vehicle and rush the core. If you are in a Scorpion, drive into the base, line up with the core and hit A. When it jumps up the ramp in front of the core, smack X and watch it do serious damage.

The iron guard card really helps because 5vs8 it's way too hard. We we're about to give up insane mode when we saw your guide.

The last level depends on the selections made throughout the chapter. My solution is not an definitive list, it is my journey. If you have made different choices, you will receive different levels.

i used the card to substract guys from their team.....WAY EASIER

I didn't use the Leviathan. Just set everyone to attack and then camped at my forward-most node. Either way it worked. Thanks for the guide. Still gave thumbs up.

I did too, used the diversion card to take away 2 of their team, set everyone on attack and ignored the Leviathan. Got it on the 2nd attempt with that set up. The key thing is to rush their core once you have control of both prime nodes.

You must complete these chapters on insane difficulty. Or... You can play up to the last level on each chapter and then quit out, select continue and change the difficulty to insane. In effect it cuts your work down a lot. It will still be quite hard though as the bots are godlike and can teleport anywhere and can dodge like hell.

NOTE: There are some parts where you can choose of two missions and depending on which order you do the missions, the final mission of each chapter WILL be different than someone who took a different path. Chapter two has a total of four different missions that could be the final one. The other chapters may also have a different selection. Follow the guide above to be safe if you're using the difficulty changing trick.
