This is how I got this achievement, "Thanks To All The Little People". This solution will only work with two controllers.
First finish the campaign by yourself, then "Replay Chapter" #1 and make it a splitscreen match. Play Chapter 1 (only one mission and it's really easy), then play the first mission of chapter 2.
After you finish chapter 2 mission 1, go back to the main menu, select Chapter V (5) and play all the missions with two controllers as well. Using this technique, you will get all 3 co-op achievements easily.
NOTE: If you do not have two controllers, find a friend to play co-op with you, or hope you can find a match online.

Um quick question, after you played chapter one and chapter two with two controllers, you just start chapter 5 and play play all mission again with two controllers right? May sound like a stupid question but it wasn't mentioned anymore if you could simply load chapter 5 ans finish it alone again to get the achievements =P

Replay the last match in the last chapter with a friend...boom, there's the achievement.

You can't select just the last match in the last chapter. You can only replay whole chapters.

Complete the campaign with someone offline or online. Just go through all the chapters if you want to get the other co-op achievements as well.
Play through and defeat Akasha at the end of chapter 5 and you will get this.
