In act 2 during the Irus fight there's a stone slab with a symbol on it on the northern wall, if you manage to knock Irus into this you'll fight her in a new area and defeating her here on MS difficulty will get the achievement. I managed to knock her through using a regular dash attack.
It's best to attempt this after completing the game and collecting souls to get the best advantage.
