How to unlock the Ghastly achievement in Untitled Goose Game - Definitive Guide

1. steal the toy plane from the boy and place it in the puddle left side on the second area (place it next to the car)
2. wait until he is there
3. approach him holding LT and untie his shoe
4. honk honk honk

he will fall into the puddle

-Now chase him over the water puddle
-While right behind him, press cn_B to trip him over the puddle

The video below is a full walkthrough of the game, including all "missable" achievements. Ghastly is unlocked by honking at the boy to chase him toward the puddle. Once in the puddle, untie his shoes and honk at him again causing him to fall.

As mentioned in The High Street description, this trophy is available the first time you enter the area so you are welcomed to attempt it on your initial run. Untie the boys shoes by nipping at them, HONK at the boy and this will send him running in the other direction. Position the event close enough to the puddle and the boy should trip and fall into the water on the left side of the area.

The bespectacled man needs to fall into a puddle. You untie his lace and start scaring him with cackling. He falls and drops his glasses. Take your glasses and, keeping a short distance from the bespectacled man, lead him into the puddle. There you return the glasses, untie the lace again and scare them again. If he falls while standing in a puddle, the trophy is yours
