On chapter 2 mission 4, when you get to the bell, hit it 3 times with the "X" button.

Vid below, I just kept hitting until it popped.

For this in the Ypres' level (Mission 4) of chapter 2 you must hit the bell 3 times.

This is during Chapter 2: Broken Earth where you play as Anna in Ypres.
You will encounter a bell puzzle which you will need to solve, one cog is visible by the far right.

The other big cog is on top of the roof which can be retrieved by climbing on the ladder to to where you will encounter a bell. bash the bell twice to drop the cog, then bash it once more, then you rang the bell 3 times and earn this Wake-Up Call Trophy!

This is during the Ypres level in Chapter 2. You will go to the top of a building where you need to drop a large bell through several floors. To do so, you'll need to find the gears and one of the gears is on top of the building. To get the gear, you need to hit the bell to shake it down. Two hits will get the gear to fall but hit the bell a third time and you will earn this trophy.

This achievement is unlocked during Chapter 2-4: Ypres

As you play through the chapter you will enter a church. To progress, you will have to climb to the bell tower, locate four gears, and ring the church bell. One of these gears is located on the bell itself. To get it, you will have to climb up to the bell and hit it. Hit the bell three times to unlock the achievement.
