There are a total of 26 costumes within the game.
NOTE: You will NOT be able to acquire all 26 within one playthrough. There are several side quests which have a decision rewarding one of two possible costumes and locking the other for that playthrough.
You will also find that most side quests require completion within their chapter; if you proceed to the next chapter the previous chapter's side quests will become locked.
I will lay out this guide in chronological order, that is, the order that you can acquire the costumes. At the end of the guide, you'll find a list ordered by how they appear in the wardrobe.
Chapter 1:
Edeni Garb (1) and Civvies (2) are available by default.
Chapter 2:
Carbon Suit (24) can be found in a blueish chest (will display a "?" when standing next to it) within the boss dungeon at the end of chapter 2.
Wraithveil (3) and Asadali Garb (4) one of these will be sent to your room depending on what you do with the item obtained at the end of chapter 2 boss dungeon.
The next two costumes are obtainable after defeating the chapter 2 boss.
The Tantini (5) is acquired upon completing the "Snare of the Naziri" side quest. This quest can be started by:

Chapter 4
Polar Bear Suit (26) can be found on top of the dwarven statue in the industrial district. From the western side of the map, head north as far as you can, then head east and follow the path round to the top.
Voltek Maker Suit (11) and Celestine Armor (12) are obtained depending on whether you use the item from Volsung in the Voltek factory.
Chapter 6:
Sylvani Warden Vestments (17) obtained from Divinia. She'll ask you to take her somewhere for the "vigil", do so and she'll give you a seed which unlocks the costume.
Sailor Lunera (25) found in a chest, go into the library, downstairs, out the door on left, then head all the way left. Chest is marked with a "?".
Laughing Gas (22) acquired on top of a rock in the temple district. At the beginning of the map there's a choice to go left or right, with some large jagged rocks in front of you. Go right, through the dungeon, then continue along the path north then west. Head south on to the rocks towards the shack type structure. The costurme is in a chest inside.
Ghost Vigilante Uniform (23) acquired by going to the northwest of the temple district and find a hidden "dungeon" on the map (a "?" will appear when walking into it), inside is a chest (and small group of coffee-loving people *hint hint*) marked with a "?" when standing next to it. The costume is inside.
Guardian Cuirass (15) and Assassin's Bodysuit (16) obtained depending on whether you kill the person or not (you'll know who I'm talking about when you get there). Spare them and talk to Celest for the cuirass, or kill them and talk to Sitara for the bodysuit.
Naziri Inquisitor Vestments (18) obtained from "The Best Defense" quest after the chapter 6 boss. Start by:

Had a problem unlocking this even with three different saves and all costumes.
Somehow my "bad" walkthrough not end saved and i played last part again to unlock.

Green Riding Hood - Chapter 3
Just had this happen to me and figured I'd add it here:
If you gave the Oksae to Gerrard at the end of Chapter 2 then Maru Han will not give you this quest item and tells you to go to the Ghost Bazaar and look there. You must go all the way back through the Entertainment District to Manduk in the Ghost Bazaar at the very end to get it and complete the quest.
Just had this happen to me and figured I'd add it here:
