Second Level in Survivor Mode: Corrupted Necropolis.
After you survive 30 minutes, you're tasked with picking up (by essentially holding the "capture point" for 30 or so seconds for each Ankh) and placing two Ankhs. Simply place the second Ankh when your run timer reads between 34:55 to 34:59. I placed it at 34:56 and got the achievement.
