Start Whiteout with the Mad Groove Arcana. It will summon the Glass Fandango to you and a set of wings.
You must get the Glass Fandango to level 8 and the wings to level 5, then find a chest to evolve.
You might need to exit the map after picking up the GF, then evolve it in a second run. The chest to evolve won't always drop in the first run that you pick it up. To increase odds banish Shadow Pinion if you see it as this also uses wings to evolve and causes a potential conflict.
You must get the Glass Fandango to level 8 and the wings to level 5, then find a chest to evolve.

I managed to evolve the Glass Fandango in my first run.
I filled up all of my slots before picking up either Wings or the Fandango. I even managed to get Wings to Level 5 before picking up the Fandango. After getting them both to top level, the game seemed to get stingy with the evolves.
It levelled up everything else before it started giving me evolves. It then went through all of my weapons that could evolve before it gave me a Candybox. I chose Lightning Ring and I had the Duplicator, but was a little skeptical I'd get the Fandango evolve. But the next chest I picked up tossed it at me. The next chest gave me the Super Candybox.
However, despite having both the Duplicator and Lightning Ring at top level, it wouldn't give me the evolve for those. Once it gave me the Super Candybox, chests just gave gold.
Not sure what to say about if this can be duplicated, I can just confirm that it can be gotten in the first run.

You on PC or Xbox? It popped no problem on console so may need to switch to that if you're on your comp

First, you must get 20 Orologions in any stage. This will unlock the new map "Whiteout".
Now go to Whiteout , when you pause your game to open the map, You will see a spear icon just north of the starting location, and you should see a spinning arrow pointer at the top of your screen. Just follow it, and you will quickly get the spear. Now you will unlock the new weapon "Glass Fandango". You can use randomazzo VIII - mad groove to make it super easy.
You can Evolve the Glass Fandango to Celestial Voulge for this achievement.

This CAN be done on any stage but is best done on Whiteout as you can use the Mad Grove arcana to immediately get both the Glass Fandango and the Wings at the start.
I recommend using Queen Sigma as this will make it significantly easier as you won't have to worry about dying and have 118 Skips/Re-rolls/Banishes.
Once the Glass Fandango reaches level 8 and the Wings are also at level 5, you can open a chest to evolve them.

I don't know why the other solution has so many more upvotes as this is a better solution. Use Sigma, Mad Grove and just grab the Glass Fandango and Wings & level them up. Took me around 8 minutes.

This Weapon Requires The Support Item "Wings" To Evolve At Max Level.
- Requires Level 5 Wings As Well, So Don't Banish Them!
Achievement Pops At The End Of The Level, After You Evolve It.

Refer to EXTRA: Glass Fandango (5G) for obtaining this new weapon. To evolve it, you need to get it to level 8 and then have the Wings at level 5, then open a chest.
